Enrolment

To enrol with Papamoa Beach Family Practice, you’ll need to complete an enrolment form. This includes a request to transfer your medical records from your current GP.

When enrolling online, you’ll be asked to upload a copy of your passport or work permit as proof of residency.

Please note: Your enrolment will need to be approved by the practice.
Completing the online form does not mean you are automatically enrolled.

You can also complete your enrolment at the medical centre, where our reception team will be happy to help.
